Cholecystoduodenal fistula and gallstone ileus - diagnosis and surgical treatment

Introduction. Gallstone ileus is a complete or partial mechanical bowel obstruction due to gallstone impaction in the lumen and most commonly occurs after stones migrate through cholecystoenteric fistula. Case outline. We present patient with signs of stone migration cholecystoduodenal fistula into duodenum hematemesis as first symptom. Conservative treatment had been started, which initially responded well. On eighth day from onset disease, condition worsened. Signs Rigler triad were identified on computed tomography magnetic resonance imaging abdomen. Enterolithotomy was successfully performed by open surgical method. Postoperative recovery orderly, without any biliary problems. Conclusion. Physical examination, upper endoscopy, radiological diagnostic procedures are complementary necessary monitoring dynamics movement deciding when perform surgery.
